READ THIS INFORMATION RIGHT THROUGH THE SECOND PAGE BEFORE COMING INTO THE OFFICE – FAILURE TO DO SO COULD RESULT IN YOUR APPOINTMENT BEING RESCHEDULED.
As the current COVID-19 outbreak continues, we are doing our best to keep everyone healthy and safe.
For your safety, please read the following guidelines put in place for all patients coming to our office. The safety of our staff and our patients is our top priority to reduce the risk of spreading COVID-19.

Patients, other than children under the age of 16, must attend the appointments ALONE, unless they need a caregiver or translator. Please DO NOT bring any family, friends or children to your appointment.

For children under the age of 16, they may be accompanied by (1) one adult or guardian.

Please use the provided sanitizer upon entering our office.

Please wear a mask.

Please arrive for your appointment at the scheduled time. If you arrive early, please wait in your car until your scheduled time.

You will complete the attached self-screening prior to you appointment and will cancel your appointment if at any point you are unable to pass this screening

You will also render a re-screening when you arrive. Any accompanying person will also be screened at the door. If you are unable to pass the screening, we will rebook you to another day.

There will be NO washroom facilities available
The measures we will be taking in the office will be:

We will complete a health screening checklist with all team members at the start of each shift.

We will provide adequate social distancing opportunities for all our patients.

We will schedule longer gaps between appointment times for cleaning.

We will continue to maintain the highest medical sanitation standards which include; using medical grade sanitizer, clean surfaces in treatment room after every patient.

We will make hand sanitizer readily available to all patients and providers.

We will assure all providers be required to practice social distancing with each other.

We will assure all providers are required to wear masks.
Since the virus is highly contagious and there is currently no vaccine for it, you will need to answer the following questions:

Have you or an immediate family had any contact with anyone with acute respiratory illness in the past 14 days?

Have you or an immediate family travelled anywhere outside of Ottawa in the past 14 days? If so, where?

Do you have a confirmed case, or have you come into contact with a confirmed case of COVID-19 in the past 2 weeks?

Do you or any immediate family member work or live in a high-risk facility – long-term care or retirement home?
Do you or any immediate family member have any of the following symptoms?

Fever

New onset of cough

Worsening chronic cough

New nasal congestion or runny nose

Headache

Muscle pains

Shortness of breath

Difficulty breathing

New sore throat
-
New difficulty swallowing
-
New decrease or loss sense of smell

Chills
-
Unexplained fatigue/malaise/muscle aches

Nausea/vomiting, diarrhea, abdominal pain

Pink eye

Please monitor yourself for symptoms. If you develop new symptoms, testing is recommended.
